Dr. Hatch sees patients at three offices of The Orthopedic Partners, an RCM Clinic. Choose whichever is most convenient; all three share one scheduling line, and your records follow you between them. Surgery is performed at Park City Surgery Center, adjacent to the Park City office, and at area hospitals.

Coming From Provo or Utah County?
Dr. Hatch does not have a Utah County office, but Heber City is a short drive up Provo Canyon and Cottonwood Heights is just past the Point of the Mountain.
Park City Surgery Center
Most of Dr. Hatch’s outpatient procedures, including arthroscopic rotator cuff repair, stabilization surgery, and many shoulder replacements, are performed at Park City Surgery Center, 800 Round Valley Drive, Park City, UT 84060, next door to the Park City office. Patients who need an overnight stay are cared for at Park City Hospital.
What to Bring to Your First Visit
- Photo ID and insurance card
- Any imaging (X-ray, MRI, CT) related to your problem, or the name of the facility where it was done
- A list of medications and treatments you have already tried, including therapy, injections, or prior surgery
- Your referral, if your insurance plan requires one
- Intake paperwork completed in the patient portal, if you received an invitation
