Our schedulers are available Monday through Friday, 8:00 AM to 5:00 PM, and can usually find you a time on the first call. It helps to have your insurance card handy, along with any recent imaging reports.

Helpful to Bring
- Insurance card and photo ID
- X-ray, MRI, or CT reports, or the name of the facility that took them
- A short list of treatments you have tried so far
- A referral, if your insurance requires one
Looking for a Second Opinion?
It is completely reasonable to want another set of eyes before shoulder surgery. Bring your images and records, and Dr. Hatch will walk you through what he sees and share a straightforward recommendation, whether that includes surgery or not.
